Referral Process
To refer someone to Ability Core, please contact us directly via phone or email. We will guide you through the referral process, which includes an initial assessment to understand the individual's needs and goals. Our team will then work with the participant to develop a tailored support plan and connect them with appropriate services.
We welcome referrals from healthcare professionals, community organisations, families, and individuals seeking support. Our goal is to ensure that everyone has access to the services they need to thrive.

Complete the referral form below and our team will review the request within 24–48 business hours. Once reviewed, our intake team will contact the participant or referrer to begin the coordination process.
